Tier 1: Investigate Other Fetid Puddles
The puddles look exactly like the one where you receive the side-quest. When you click on a puddle, you will get flies flying around you and a debuff to confirm it. You need to click on 5 puddles in a row without the debuff going away in order to advance the mission to Tier 2. The best way to do this is to clear out the area around puddles ahead of time. This way, you can just run from puddle to puddle clicking them without having to worry about aggroing enemies that may interrupt you while clicking on puddles.
Tier 2: Defeat The Forest Heart’s Decay
When you click on the 5th puddle, the Forest Heart’s Decay will appear. Defeat him to advance to Tier 3 of the mission.
Tier 3: Tell Milosh About The Insect Golem
Simply walk over to Milosh and click on him to tell him about the Insect Golem. This will end the mission for you.
